Using 1&1/2 inch thick, 2 inch nominal, lumber to cut the holes out of will leave a 1/2 inch of extension box below the bottom of the wood block. The canopy will conceal that last 1/2 inch. Make the cuts to where the dw board will eventually slide up and over the box, and then it will bottom out against the ceiling joists (or existing structure). (at 100%) i don't know whether it is not to scale, or whether they are counting on saw kerf to make it 4 inches.

I know that they have templates that guide spiral saws to cut drywall for single gang and double gang boxes, but don't remember whether they offered printable templates. Cut the box out while not getting over a 1/8 in or so from the outer sides of the ceiling box. Anything sized properly to slip over the mounting screws and fill the space between the recep and the box. Remove the deck ears from the box used as the extension ring so that they won't interfere with the installation of the canopy.
